穴位温灸加中药灌肠治疗慢性肾脏病3～4期的临床研究

目的观察穴位温灸加中药灌肠治疗慢性肾脏病3～4期的临床疗效。方法将78例慢性肾脏病3～4期患者随机分为2组。在控制饮食及症治疗的基础上,治疗组42例予穴位温灸加中药灌肠,每日1次;对照组36例予我科协定处方济生橘皮竹茹汤加减,每日1剂,早晚各1次服。疗程均为12周。观察2组治疗前后血尿素氮（BUN）、血清肌酐（Cr）、内生肌酐清除率（Ccr）、血清白蛋白（Alb）、血清总蛋白（TP）、血红蛋白（Hgb）及中医证候积分的变化,比较2组临床综合疗效。结果治疗后2组临床综合疗效比较差异无统计学意义（P〉0.05）;治疗后2组中医证候积分与治疗前比较差异均有统计学意义（P〈0.05）,2组间比较差异也有统计学意义（P〈0.05）。治疗后2组Cr明显上升（P〈0.05）,Ccr明显下降（P〈0.05）,组间治疗后比较差异也有统计学意义（P〈0.05）;2组治疗后Hgb明显上升（P〈0.05）,但组间治疗前后差值比较差异无统计学意义（P〉0.05）;2组其余实验室指标治疗前后及组间比较差异无统计学意义（P〉0.05）。结论穴位温灸加中药灌肠治疗慢性肾脏病3～4期临床疗效肯定,值得推广应用。

Clinical research of point moxibustion combined with Chinese herbs enema treatment on chronic kidney dis-ease phase three to four

Objective To evaluate the point moxibustion combined with Chinese herbs enema treatment on chronic kidney disease phase three to four. Methods 78 patients were divided into treatment group and control group by a randomized, parallel - group clinical trial. On the basis of the control diet and symptomatic treatment, 42 cases in treatment group received the point moxibustion combined with Chinese herbs enema treatment, once a day. 36 cases in the control group received Jishengjupizhuru decoction, once a day. The course of treatment was 12 weeks in two groups. Tile changes of creatinine, serum creatinine clearance （Ccr） , blood urea nitrogen（ BUN ） , hemoglobin （Hb）, albumin（Alb）, total protein and traditional Chinese medicine（TCM） syndrome integral before and after treatment were observed. Results After treatment, there was not statistically significant on the clinical effects（ P 〉 O. 05 ） ; There was significant difference between before and after treatment on traditional Chinese medicine syndrome total score in two groups（ P 〈 0.05）. There was obvious difference between two groups on traditional Chinese medi- cine syndrome total score after treatment（ P 〈 0.05）. The levels of Cr increased significantly in the treatment group and control group （ P 〈 0.05 ）, and Ccr decreased significantly （ P 〈 0.05 ）. There was difference between two groups and before and after treatment （ P 〈 0.05 ） ; in the treatment group and control group Hb rose significantly （P 〈 0.05）, but there was no difference between two groups （ P 〉 0.05 ） ; there was no significant difference be- tween two groups on other laboratory indicators before and after treatment（ P 〉 0.05 ）. Conclusion Point moxibus- tion combined with Chinese herbs enema treatment has certainly clinical efficacy and should be widely applied.
