主动脉夹层动脉瘤诊断和治疗的临床研究

目的：探讨主动脉夹层动脉瘤的临床特点、诊断和治疗措施。方法：回顾分析11例主动脉夹层动脉瘤患者的临床资料,包括基础疾病、临床表现、实验室检查、治疗方法和预后。结果：11例主动脉夹层动脉瘤患者的基础疾病包括高血压、糖尿病、马凡氏综合症和妊娠;主要临床表现有胸痛、心悸、气短,血压升高、心动过速、心脏杂音;胸片、CT扫描、超声心动图可见主动脉扩张、主动脉内膜撕裂、出血等。11例患者均采用内科保守治疗,主要治疗措施是控制血压和心率,其中3例死亡,1例转院,7例存活。结论：主动脉夹层动脉瘤发病急、病情进展快、死亡率高,早期诊断和治疗是降低死亡率的关键。

CLINICAL STUDY ON DIAGNOSIS AND TREATMENT OF AORTIC DISSECTING ANEURYSM

Objective：To investigate the clinical characteristics,diagnosis and treatment of aortic dissecting aneurysm.Methods：The clinical data including basic diseases,clinical manifestation,laboratory examination,treatment and prognosis of 11 aortic dissecting aneurysm patients were retrospectively analyzed.Results：The basic diseases of 11 cases aortic dissecting aneurysm included hypertension,diabetes mellitus,Marfan＇s syndrome and pregnancy.The main clinical manifestations were chest pain,palpitation,shortness of breath,elevation of blood pressure,tachycardia and cardiac souffl e.Chest X-ray,CT and ultrasonic cardiogram showed aorta dilation,aortic intimal tear and bleeding.All the 11 patients were all treated with medicine only,the main treatment measures were controlling blood pressure and heart rate.Then,3 patients died,1 patient transferred to other hospital,and the other 7 patients stayed alive.Conclusions：The characteristics of aortic dissecting aneurysm are acute onset,speedy development and high lethal rate;early diagnosis and treatment is the key to reduce mortality.
