早期肠内营养在脑外科手术后患者中的临床价值

摘    要:目的:评价早期肠内营养对脑外科患者手术后的临床价值。方法选取2010年1月~2013年12月脑外科手术后早期应用肠内营养的患者33例作为观察组,选取同期未应用肠内营养的脑外科术后患者32例作为对照组,观察患者术前1天、术后第1d、第1周及第2周血清总蛋白、血清白蛋白、氮平衡,术后第2周患者上臂肌围及肱三头肌皮褶厚度变化,分析患者的营养状况、术后并发症发生情况等指标。结果术后1周、2周观察组患者血清总蛋白、白蛋白、氮平衡值明显高于对照组(P<0.05),观察组上臂肌围及肱三头肌皮褶厚度明显优于对照组(P<0.05);术后2周,观察组患者并发症发生率明显小于对照组(P<0.05)。结论早期肠内营养可以改善脑外科手术的营养状态,降低患者并发症的发生率,提高临床治疗效果。

Abstract:Objective To evaluate the clinical value of enteral nutrition on the neurosurgical patients at early stage of brain surgery. Methods From Jan 2010 to Dec 2013, 65 cases of neurosurgical patients were randomly divided into two groups:treatment group(33 cases) and control group(32 cases). The patients in treatment group were administrated with enteral nutrition at early stage after operation. While the patients in the control group were administrated with exteral nutrition at the same period. The variations of serum total protein, serum albumin and nitrogen balance were recorded on the first,seventh and fourteenth day after operation and on the first day before operation. And on the fourteenth day after operation , the change of pa-tients' arm muscle circumference (AMC)and triceps skinfold thickness (TSF)were recorded and analyzed. Results On the first,seventh and fourteenth day after operation, the serum total protein, serum albumin and nitrogen balance of the treatment group were significantly higher than those of control group, and the difference had statistical significance (P<0.05). The AMC and TSF of the treatment group were obviously better than those of the control group(P<0.05). On the fourteenth day after opera-tion, the ratio of complications in treatment group was significantly lower than that in the control group (P<0.05), and the dif-ference had statistic significance (P<0.05). Conclusion Enteral enteral nutrition on the neurosurgical patients at early stage can improve the patients' nutritional state and decrease the incidence of complications.
