神经导航辅助下甲介型蝶窦垂体微腺瘤的经蝶入路手术

目的：介绍神经导航辅助下甲介型蝶窦垂体微腺瘤经蝶手术。方法：应用StealthStation神经导航系统指导经碟手术入路，使用高速磨钻靡除蝶窦内骨质。结果：10例甲介型蝶窦垂体微腺瘤达到全切除，无手术并发症。10例平均坐标误差1．14－3．91mm(平均2．30mm)，术前准备时间为9－15min(平均10．3min)，手术时间为90－140min(平均120min)，结论：神经导航提供实时三维定位，指导甲介型蝶窦垂体微腺瘤的经蝶手术，疗效满意。

Neuronavigator-guided Resection of Pituitary Microadenoma via Transsphenoidal Approach in Patients with Conchiform Sphenoidal Sinus

Objective To introduce the experience of transsphenoidal surgery for pituitary microadenoma in the patients with conchiform sphenoidal sinus. Methods Pituitary microadenomas of the 10 patients with conchiform sphenoidal sinus were resected via transsphenoidal approach under guidance of the StealthStation neuronavigator, and high-speed drill was used to remove the superfluous bone in the sphenoidal sinus. Results The mean fiducial error was ranged from 1.14 to 3.91 ram (mean, 2.30 mm)during the surgery. The time used for manipulation of the neuronavigator and operative time were 9 to 15 min (mean, 10.3 min) and 90 to 140 min (mean, 120 min), respectively. Total tumor removal was achieved in all patients, and endocrine symptoms were improved in the 8 patients. There was no complication attributable to inaccurate localization from the neuronavigational system. Conclusion With the help of StealthStation neuronavigator which can provide realtime localization with three-dimensional information, combined with the use of high-speed drill, the pituitary microadenoma can be successfully resected via the transsphenoidal approach in the patient with conchiform sphenoidal sinus.
