免疫标记物在恶性腹腔积液细胞学诊断中的应用

摘    要:目的探讨免疫标记物在腹膜恶性间皮瘤(PMM)和转移性卵巢浆液性腺癌(OSA)鉴别诊断中的作用。方法应用细胞包埋技术及免疫细胞化学法检测PMM20例和OSA35例患者腹水中AE1/AE3、Vimentin、CK5/6、Calretinin、WT.1、HBME-1、CK7、CK20、CEA的表达。并对其阳性表达率及诊断价值进行分析比较。结果Calreti—nin、CK5/6、WT-1、HBME-1、CK7和CEA在PMM与OSA中的表达差异有统计学意义(P〈0.05),AE1/AE3、Vimentin和CK20的表达差异无统计学意义(P〉0.05)。诊断PMM最敏感的抗体是AE1/AE3(100.0%)和Calretinin(95.O%),其次是CK5/6(90.0%)和WT-1(90.0%),特异性最高的抗体是Calretinin(94.3%)和CK5/6(94.3%);对OSA敏感性最高的抗体是AE1/AE3(100.0%)和CK7(100.0%),其次是CEA(88.6%),特异性最高的抗体是CEA(95.0%)和CK20(95.0%)。结论Calretinin、CK5/6和CEA兼具特异性和敏感性,可作为PMM和OSA鉴别诊断中的首选抗体;AE1/AE3和Vimentin阳性的细胞,结合细胞的形态学特点可以初步区分PMM和OSA。

Abstract:Objective To investigate the immunological markers in peritoneal malignant mesothelioma (PMM) and metastatic ovarian serous adenocarcinoma (OSA) in the differential diagnosis role. Methods Using cell embedding technique and immunoeytochemical assay PMM 20 examples and OSA 35 patients, ascites AE1/AE3, Vimentin, CK5/6, Calrefinin, WT-1, HBME-1, CK7, CK20, CEA expression. And its positive expression rate and diagnostic value were analyzed and com- pared. Results Calrefinin, CK5/6, WT-1, HBME-1, CK7 and CEA in PMM with OSA expression difference was statistically significant ( P 〈 0.05 ), AE1/AE3, Vimentin and CK20 expression was no statistical difference significance ( P 〉 0.05 ). PMM most sensitive diagnostic antibodies AE1/AE3 (100.0%) and Calretinin (95.0%), followed by CK5/6 (90.0%) and WT-1 (90.0%), specificity of antibodies Calretinin (94.3 % ) and CK5/6 (94; 3 % ) ; highest sensitivity for OSA antibodies AE1/AE3 ( 100.0% ) and CK7 ( 100.0% ), followed by the CEA (88.6%) , specificity of antibodies is CEA (95.0%) and CK20 (95.0%). Conclusion Calretinin, CK5/6 and CEA are good at specificity and sensitivity, as PMM and differential diagnosis of OSA preferred antibodies ; AE1/AE3 and Vimentin positive cells, combined with the morphological characteristics of cells can initially distinguish PMM and OSA.
