转移性肺肿瘤的X线诊断

目的：通过对肺转移瘤基本X线改变的分析。提高对肺转移瘤的认识，为临床制订合理的治疗方案提供必要的依据。方法：收集海宁市第二人民医院放射科98年1月至2000年6月拍摄胸片的37例其原发癌瘤，经病理（18）、细胞学（3）CT、MRI、特殊造影检查（16）等确诊的肺转移瘤资料，对其肺部转移性美丽 灶的基本X线表现、原发癌肿来源、病理类型等进行分析研究。结果：肺转移瘤属肺内转移10例、相邻肺组织器官转移18例，不相邻肺组织器官转移9例，多见的基本X线表现有栗粒样阴影、小结节絮状阴影、大结节团块样阴影、园形或类园殂阴影和混合型，多种来源的癌肿肺部转移灶可表现有共同特征，而同一种原发病灶又可呈现多种类型的肺部转移灶，且转移灶发展到一定程度时，往往是混合型的X线表现。结论：肺转移瘤的表现为多源性、多样性和多变性。肺转移瘤病例还常伴有胸膜及肺间质改变。

The X-ray diagnosis of lung metastatic tumor

Objective:To analysis the X-ray basic change on the lung metastatic tumor,improve recognition of lung metastatic tumor.It was formulated rational treatment scheme for clinical.Methods:WT5BZ]Thirty-seven cases with primary tumour in department of X-ray ,Haining second hospital January 1998 to June 2000,all patients with pathology (18 cases),cytology(3 cases)and computed tomography (CT),magnetic resonance imaging(MRI),specific photology etc.(16 cases)proved lung metastatic tumor and to studied the X-ray basic change,primary tumuorign and patholog types etc.Results:Ten of 37 cases were lung-iner metastatic,eighteen adjacet to organic metastatic and nine non-adjacet to orgnic metastatic.Common X-ray basic magnification,miliary shadow,small node patch shadow,large node mass shadow,circular of circular-like shadow and nrix type.The lung metastases focus of nrultiregion cancer were manifested in common character.Both identical primary focus and appeared lung metastases focus of multiple types,metastases focus were developed often emerged X-ray manifestaion of blend type.Conclusion:The lung metastases tumor of manifestation often multisouce, pleonvorphic and polytropy.The lung metastases tumor usually accompany pleura and lung interstitial change.
