准分子激光原位角膜磨镶术治疗近视并发症

摘    要:目的 分析准分子激光原位角膜磨镶术 (LASIK)治疗近视的并发症 ,探讨其发生的原因、预防和处理措施。方法  72 3例 ( 1390只眼 )近视患者 ( - 1 5 0~ - 2 2 0 0D)行LASIK术后随访 12个月 ,观察其术中、术后并发症。结果 术中角膜瓣形成不全 5只眼 ( 0 36 % ) ,角膜瓣过薄 4只眼( 0 2 9% ) ,角膜瓣游离 6只眼 ( 0 4 3% ) ,角膜瓣层间异物残留 12只眼 ( 0 86 % ) ,屈光回退 >1 0D者 2 0只眼 ( 1 4 4 % ) ,激素性高眼压 16只眼 ( 1 15 % ) ,眩光和夜驶困难 2 4只眼( 1 73% )。结论 LASIK治疗近视安全、有效 ,但仍存在一定的并发症 ,必须引起重视 ,提高手术技巧和完善手术设计有助于进一步减少手术并发症的发生和提高手术的预测性和准确性

Abstract:Objective To analyze the complications of excimer laser in situ keratomileusis(LASIK) for myopia and investigate the causes, prevention and treatment of these complications. Methods The SCMD microkeratome and Schwind Keratom excimer laser were used to correct myopia on 1 390 eyes in 723 cases. All of them were followed up for 12 months to observe and analyze the complications. Results The complications included incomplete flap 5 eyes(0 36%) ,thin flap 4 eyes (0.29%), free flap 6 eyes (0 43%),foreign body in the interface 12 eyes (0 86%), refractive regression>1 0 D 20 eyes(1 44%), postoperative steroid intraocular hypertension 16 eyes(1 15%), night glare 24 eyes(1 73%). Conclusion LASIK is a safe and effective method for correcting myopia. Yet attention should be paid to the complications during intra operation and post operation. With the improvement in operative technique and better designed procedure,these complications can be reduced and the pre operative correction in estimation of postoperative results and accuracy in the correction of myopia can be increased.
