摘    要:目的探讨超声引导下标准通道经皮肾镜超声碎石术治疗嵌顿性输尿管上段结石的安全性及临床疗效。方法回顾性分析我院2008年1月至2011年12月期间在超声引导下应用标准通道经皮肾镜超声碎石术治疗98例嵌顿性输尿管上段结石患者的临床资料。结果98例患者均采用单通道碎石一期完成手术,输尿管结石清石率为97.96%,平均手术时间(72±25)min,术中平均失血量(85±67)ml。1例患者术中扩张通道时出血形成肾周血肿,经保守治疗后治愈。余未发生重大并发症。结论超声引导下标准通道经皮肾镜超声碎石术治疗嵌顿性输尿管上段结石具有疗效确切、高效安全、并发症少等优点,值得临床推广应用。

Percutaneous nephrolithotomy through a standard channel with ultrasonic power under B-type ultrasound guidance for the treatment of 98 cases with impacted upper ureteral calculi

Abstract:Objectives To evaluate the efficacy and safety of standard percutaneous nephrolithotomy (PCNL) with ultrasonic power under B-type ultrasound guidance in treating impacted upper ureteral calculi. Method The clinical data of 98 patients with impacted upper uretetal calculi, who were treated with standard percutaneous nephrolithotomy with ultrasonic power under B- type ultrasound guidance in our hospital from January 2008 to December 2011 were retrospectively summarized. Result All patients were treated with single tract in one stage. The stone-free rate was 97.96%, and the average operative time was (72±25)min. The average blood loss was (85±67)ml. One case who suffered from intraoperative bleeding with puncture and dilation was cured by conservative management. No significant complication were found in the other cases. Conclusion It is worth of being spread of standard PCNL with ultrasonic power under B-type ultrasound guidance treating impacted upper ureteral calculi, in consideration of its advantages of few complication and high stone free rate.
