MRI在鼻咽癌诊疗中的价值

摘    要:目的探讨MRI在鼻咽癌诊疗中的应用价值。方法经病理证实的鼻咽癌72例,MRI扫描采用Marconi eclipse 1,5T超导磁共振成像系统,所有病例行横断位和冠状位T1WI,横断位和矢状位T2WI,脂肪抑制序列T2WI横断位,24例在平扫后行增强扫描。结果72例鼻咽癌MRI均见长T1长T2信号鼻咽部肿块,其中67例可见咽旁间隙侵犯,47例可见颅底骨质信号改变,6例侵犯蝶窦,14例可见海绵窦内软组织影,3例累及后颅窝,61例可见颈部淋巴结转移。结论MRI可以清晰显示鼻咽癌肿块、邻近结构的侵犯和颈部淋巴结转移,在鼻咽癌的诊断和分期中有重要价值。

Abstract:Objective To investigate the value of MRI using in the nasopharyngeal carcinoma (NPC).Methods Seventy two cases of NPC patients proved with pathology underwent MRI examination with super conducting magnetic system(Marconi eclipse, 1.5T). Routine SE and FSE sequence with axial , sagittal and coronal slices were scanned. FSE T2WI with fat saturation sequence was used. Contrast enhance scan were performed in 24 cases. Results The soft tissue mass of nasopharyngeal cavity were showed by MRI in all 72 cases. 67 cases were displayed parapharyngeal space involvement, the skull base erosion were displayed in 47 cases. 6 cases with sphenoid sinus invasion and 14 cases cavernous sinus invaded were detected. 61 cases had cervical lymph node metastasis. Conclusion MRI is able to reveal the invasion extent into the structures around the tumor mass accurately, to easily visualize the early erosion of skull base and intracranial invasion of NPC. It is of great value to applicant MRI into the diagnosis and staging of NPC.
