白细胞分类计数对婴幼儿腹泻病的诊断价值

目的:探讨白细胞分类计数对婴幼儿腹泻病的诊断价值。方法:采用常规白细胞计数方法得出各病例组测定值,确定有意义界值后得出各项的敏感性和特异性。结果:重型腹泻组中性粒细胞杆状核细胞数和B/N值明显高于非腹泻对照组和轻型腹泻组(P<0.01),中型腹泻组B/N值也高于非腹泻对照组(P<0.05)。各病例组间白细胞总数、淋巴细胞和中性粒细胞分叶核细胞增加并无差异(P>0.05)。中性粒细胞杆状核细胞数和B/N值对婴幼儿重型腹泻组的诊断敏感性分别为81.71%和84.76%,特异性分别为75.61%和78.05%。两项共同诊断的敏感性和特异性达85.98%和79.68%。结论:白细胞计数总数和淋巴细胞的增加对婴幼儿腹泻病的诊断意义不大,中性粒细胞杆状核细胞绝对数和B/N值确有重要价值。B/N值>0.10应考虑是由重要致病微生物引起。

Diagnostic Value of Leucocyte Differential Count to Infantile Diarrhea Disease

Objective:To explore the diagnostic value of leucocyte differential count to infantile diarrhea disease.Methods:The results of WBC and differential count were analysed and then,the susceptible and specific data were measured after the borderline determination.Results:Significant differences were observed in Neutrophilic band form and B/N value between heavy group and light group(P<0.01),and B/N value of medium group was much different from the comparison group(P<0.05).No significant differences were observed in WBC、lymphocyte and segmented neutrophil of all groups(P>0.05).The sensitivity and the specificity of neutrophilic band form and B/N value to the heavy group was 85.98% 、84.76% and 75.61% 、78.05% respectively and that would reach 85.98% and 79.68%.when they had been used to diagnosis together.Conclusion: There no diagnosis value of WBC and lymphocyte to infant diarrhea disease, but neutrophilic band form and B/N value are important for the infant diarrhea diagnosis.Infantile diarrhea disease is likely to be caused by important microbe.When B/N>0.10.
