冠心病患者血清胆红素和尿酸水平的变化及临床价值

摘    要:目的探讨冠心病(coronaryheartdisease,CHD)患者血清胆红素和尿酸水平的变化及在疾病康复过程中发挥的作用。方法研究对象均为2002年至2004年在邯郸市中心医院心内科行冠状动脉造影阳性的患者,对照组为同期冠状动脉造影阴性者。所有研究对象禁食12~16h于次晨空腹抽取静脉血,AU2700全自动生化分析仪检测,胆红素测定采用钒酸盐法,尿酸测定采用酶比色法。观察CHD组和对照组血清中胆红素和尿酸的变化规律。结果CHD患者血清中的胆红素含量低于对照组(P<0.05),CHD组的尿酸水平明显高于对照组(P<0.01),男性尿酸水平明显高于女性(P<0.05)。结论血清胆红素偏低和尿酸水平升高都是CHD的危险因素,动态检测血清胆红素和尿酸的水平对CHD的诊断、治疗和预后评估具有重要的临床意义。

Abstract:Objective To investigate the changes of serum bilirubin and uric acid in patients with coronary heart disease(CHD)and its clinical application. Methods The subjects in this study were all positive in coronary arteriography in department of cardiology, Handan central hospital from 2002 to 2004. As control group, the ones were negative in coronary arteriography homochronously. Blood samples were collected from peripheral vein after an overnight fast of 12 to 16 hours. The levels of serum bilirubin and uric acid were measured on AU2700 full automatic biochemistry analyzer. Results The changes of serum bilirubin and uric acid were observed between CHD group and the control group (P< 0.05). The levels of serum bilirubin were lower in CHD group than those in the control group (P< 0.05). The levels of serum uric acid were much higher in CHD group than those in the control group (P< 0.01), and the levels in males were much higher than the levels in females (P< 0.05). Conclusion High serum bilirubin and low uric acid were both risk factors for coronary heart disease. The dynamic determination of serum bilirubin and uric acid may contribute to the diagnosis, therapy and prognosis in CHD patients.
