宫颈癌组织中CD40分子表达的临床意义及其与肿瘤血管生长的相关性

目的研究CD40、人乳头瘤病毒（HPV）、血管内皮细胞生长因子（VEGF）及CD34在不同宫颈组织上的表达及其对宫颈癌肿瘤血管生成的影响。方法收集239例子宫颈组织标本及临床病理资料,其中正常子宫颈组织38例,慢性宫颈炎43例,宫颈上皮内瘤变（CIN）102例,宫颈鳞状上皮癌标本56例。采用免疫组织化学链霉菌抗生素蛋白过氧化物酶连接（SP）法检测不同宫颈组织中CD40、HPV16/18-E6、VEGF的表达以及以CD34抗体检测宫颈癌的微血管密度（MVD）。结果 CD40分子、HPV16/18-E6、VEGF和CD34＋MVD在宫颈癌中均为高表达,与正常宫颈组织及慢性宫颈炎组织相比,差异均有统计学意义（均P〈0.05）。CD40分子的在宫颈癌组织中的表达与HPV16/18-E6、VEGF以及CD34＋MVD的表达成正相关（P〈0.01或〈0.05）。CD40分子在宫颈癌中的表达和肿瘤有无淋巴结转移、肿瘤肌层浸润深度亦成正相关（P〈0.01或〈0.05）。结论 CD40分子在宫颈癌肿瘤发生、发展过程及肿瘤血管的生成中起着一定的作用;CD40分子在宫颈癌组织上的表达不仅为研究肿瘤的临床进展提供了参考指标,同时也为宫颈癌的肿瘤生物学治疗提供了一靶分子。

Clinical significance of the expression of CD40 on human cervical carcinoma and its correlation with neovascularization

Objective To study the expression of CD40 and associated molecules human papilloma virus（HPV）,vascular endothelial growth factor（VEGF） and CD34 on different types of malignant cervical cancer,and to study the effect of CD40 on the angiogenesis of cervical cancer.Methods Surgical biopsy specimens were obtained from 239 patients（cervical squamous epithelium cancer 56 cases,cervical intraepithelial neoplasia 102 cases,chronic cervical inflammation 43 cases and the normal control 38）.The expression of CD40 and associated molecules HPV,VEGF and CD34 microvessel density（MVD） on different types of malignant cervical cancers were examined by SP.The relation of CD40 and associated molecules and clinical significance were analyzed.Results There were significant differences between expression of CD40,HPV,VEGF and CD34＋MVD on cervical cancer and on the normal controls（P〈0.05）.There were a positive correlation between CD40 expression and HPV,VEGF CD34＋MVD（P〈0.01 or 〈0.05）.Furthermore,There was a close correlation between the expression of CD40 and lymph nodule metastasis and the invasive extent of muscular layer（P〈0.01 or 〈0.05） in cervical cancer.Conclusion CD40 may be involved in neovascularization of cervical carcinoma,and CD40 and VEGF may be useful biomarkers for evaluating the risk of developing cervical carcinoma,and may also be used as a target for therapy.
