额部皮瓣在修复上唇缺损中的应用

目的：探讨额部皮瓣在修复上唇缺损中的应用。方法：对13例各种原因引起的上唇缺损的病例采用额部皮瓣为主配合鼻唇沟瓣的治疗方法进行修复。结果：术后皮瓣血运良好,未见感染征象,12例皮瓣全部成活,Ⅰ期完成修复;1例皮瓣部分坏死,经Ⅱ期皮片部分移植后愈合。随访6个月-3年,转移后皮瓣质地优良,外观及功能满意,有部分感觉恢复。结论：在切取额瓣时由于蒂部过长而宽度不够时,术后可出现动脉血供充足,静脉回流受阻而出现瘀血,导致肌皮瓣成活欠佳现象。

Experience of using frontal flap in repairing of upper lip defects

Objective：To evaluate the usage of frontal flap in reconstruction of upper lip defects.Methods：Thirteen cases with various cases of upper lip defects were reconstructed by forehead flap based treatment with the nasolabial fold flap repair.Results：Flaps with blood supply are good,no patients is infected,the flaps totally survived by a complete repair in 12 patients.One case of flap is partial necrosis,the flap survives by skin graft.Their appearance and function were satisfactory after an average of a year follow-up（ranged from six months to three years）.The texture of the flaps were excellent.The sensation of the flaps recovered in different degree.Conclusion：With cutting frontal flap,the Length of the pedicle should be moderate,the width of the pedicle should be enough.If not arterial blood supply sufficient and an venous obstruction cause blood stasis,leading to the phenomenon of poor muscle flap.For example one cases of flap necrosis is caused by this factor.
