亚低温治疗对重度颅脑损伤的临床疗效

本文旨在探讨亚低温治疗对重度颅脑损伤的临床疗效。实验对象选自２４例重度颅脑损伤患者，其ＧＯＳ评分≤８。将以上患者随机分为低温组和常温组各１２例，低温组以冰毯机将肛温尽快降至３２～３５℃，同时给予冬眠合剂Ⅰ号，亚低温疗程为１周，每日监测平均动脉压（ＭＡＰ）及脉搏（Ｐ），于第３日及第７日腰穿测定颅内压（ＩＣＰ），采血样作血气分析，并测定肌酸磷激酶（ＣＰＫ）及血小板（ＰＬＴ），于第７日进行ＧＯＳ评分。常

Treatment for severe head injury with mild hypothermia and artificial hibernation

The curative effect of mild hypothermia on severe head injury was studied. The sample consisted of 24 patients with severe head injury. GCS of each patient was not nore than 8. The patients were divided into mild hypothermia group and northermia group at random (12 patients in each group).The recod temperature of patients in mild hypothermia group were cooled to 32-35T by cooling blanket.Cocktail Hibernation I were given to patients. The curative period of mild hypothermia was l week.The MAP and P were monitored every day .The ICP was detected by lumbar puncture on the 3rd and 7th day .The blood sample was analyzed .The CPK and PLT were determined. GOS were recouled on the 7th day.The noIthermia group was managed as same as the mild hypothermia group except cooling.The results showed that mortality of mild hypothermia gnup (25 % ) was significantly less than that of northermiagroup(66. 6%' P < 0. 05). GOS of mild hypothermia group was greater than that of northermia group. ICP decreased more significandy in mild hypothermia group than in northermia group. So did CPK and PLT.The blood potassium, sodium, PCO, and PO, did not displayed apparent difference between two troups.MAP was lower in mild hypothermia group. P was about the sarne in the two groups. It indicated that mild hypothermia can significanily lower mortality of severe head injury and improve the prognosis .It can also significantly decrease the ICP and CPK.
