肝外胆道梗阻性疾病的MRC诊断

目的:评价磁共振胆管成像(MRC)对肝外胆道梗阻性疾病的诊断价值。材料与方法:54例经手术(n=33)、活检(n=2)及临床随访(n=19)证实患有肝外胆道梗阻性疾病的患者于philips Gyroscan T5NT超导磁共振扫描仪上采用不屏气重T_2加权Turbospine echo脉冲序列行MRC检查,并对MRC在判断梗阻存在情况及其水平.以及在鉴别良、恶性的准确性方面进行了前瞻性分析。结果:MRC对梗阻存在及梗阻水平的判断敏感性达到100％,对良性、恶性梗阻的鉴别诊断准确率达95％。结论:MRC能无损伤地显示肝外胆道梗阻性疾病胆系解剖结构并可提供重要的形态学信息,可作为肝外胆道梗阻疾病的常规诊断方法。

MR Cholangiography Diagnosis of Extrahepatic Biliary Obstructive Diseases

Purpose: To assess the diagnostic value of MR cholangiography in extrahepatic biliary obstructive diseases. Materials and Methods: Fifty-four patients with extrahepatic biliary obstrucion proven by surgey(n = 33), biopsy (n = 2) and clinical follow - up(n = 19), underwent non - breath hold heavily T2 weighted Turbo -Spin echo MRC in a Philips Gyroscan T5NT. The sensitivity of MRC in determining of the existence and the level of obstruction and the accuracy of MRC in differentiating the malignancy from benignancy of obstructive diseases were prospectively analyzed. Results: MRC clearly displayed the biliary anatomical structures during extrahepatic biliary obstructive diseases; the sensitivity of MRC in determining the existence and the level of obstruction reached to 100%; the accuracy of MRC in differentiating malignancy from benignancy was 95%. Conclusion: MRC can clearly display the biliary anatomical structures without any invasion and can provide significant morphological informations, therefore, it can be taken as a conventional diagnostic method of extrahepatic biliary obstuctive diseases.
