Sema4D和HER-2在乳腺癌组织中的表达及临床意义

摘    要:目的探讨Sema4D和人类表皮生长因子受体2(HER-2)在乳腺癌组织中的表达及临床意义。方法采用免疫组织化学sP法检测110例乳腺癌组织中Sema4D和HER-2的表达,并检测肿瘤微淋巴管密度(Microlymphtic vessel density,MLVD)。结果乳腺癌组织中Sema4D和HER-2蛋白的阳性表达率分别为71.82%(79/110)和33.64%(37/110),与对照组比较差异有统计学意义(P〈0.01):淋巴结转移阳性组的Sema4D阳性表达率和MLVD值显著高于淋巴结转移阴性组(P〈0.01);Sema4D和HER-2阳性表达与组织学分级、TNM分期、淋巴结转移及ER表达状态显著相关(P〈0.05),而与肿瘤大小及PR表达状态无关(P〉0.05)。相关分析结果显示Sema4D与HER-2表达呈正相关(r=0.535,P〈0.01);单因素生存分析显示,Sema4D高表达组的5年生存率(74.19%)高于低表达组(53.16%),其差异有统计学意义(P〈0.05)。结论Sema4D可能对乳腺癌的发生、发展起重要作用。

Abstract:Objective To investigate the expressions of Sema4D and HER-2 in breast cancers and their relationship with microlymphtic vessel density ( MLVD ). Methods MLVD and expressions of Se- ma4D and HER-2 were detected in 110 cases of breast cancer tissues by immunohistochemistry. The rela- tionship of Sema4D and HER-2 expressions with elinicopathologic parameters was analyzed. Results The positive expression rate of Sema4D and HER-2 was 71.82% (79/110) and 33.64% (37/110) , respective- ly ,with a statistically significant differences compared to those in the control group ( P 〈 0. 01 ). The posi- tive expression rates of Sema4D and MLVD in cases with lymphatic metastasis were higher than that without lymphatic metastasis ( P 〈 0. 01 ). The expressions of Sema4D and HER-2 were closely correlated with the histological grade, TNM stage, lymph node metastasis, and expression of ER of breast cancers ( P 〈 0. 05 ) , but were not related to tumor size and expression of PR ( P 〉 0. 05 ). On univariate analysis, the disease-free survival rate of patients with Sema4D positive expression was better than those with its negative expression ( P 〈 0. 05 ). Sema4D expression was positively correlated with the HER-2 expression ( r = 0. 535, P 〈 0. 01 ). Conclusions Sema4D may play important roles in the carcinogenesis and development of a breast cancer.
