外伤性肝破裂76例诊治体会

目的:总结腹部外伤肝破裂的个性化处理方案.方法:对76例不同年龄、不同致伤原因、不同肝损伤部位、不同肝损伤程度的腹部外伤肝破裂患者进行以手术为主的综合治疗,依一般状况、肝损伤部位及肝损伤程度的不同而实行个性化手术方案,方法包括:在正确诊断的前提下,在抢救休克的同时尽早手术.术式有肝单纯缝合、肝动脉结扎、部分肝切除术及填塞法.建立充分引流,术后积极处理并发症.结果:患者大多取得了良好的治疗效果.结论:对于腹部外伤肝破裂的病例,手术方式应采取个性化治疗方案.

The Diagnosis and Treatment of Liver Traumatic Rupture in 76 cases

Objective:To summarize the individual therapeutic regimens for the cases of liver traumatic rupture. Methods:In this study, with different age , traumatic cause ,pars affecta and degree of injury, 76 cases of liver traumatic rupture received combined therapy. In the therapies , operations were main regimen . Individual operativetherapeutic regimens were taken depending on the vital signs, pars affecta and degree of injury. In precondition of correct diagnosis, rescue on shock should be taken immediately. Simultaneously, operations were undertaken as early as possible. The methods of operations : liver simple suture, hepatic artery ligation, partial hepatectomy and liver plugging. Sufficient drainages were established in the process of operations . Results: most of patients had a satisfied therapeutic efficacy. Conclusion:The individual therapeutic regimens of operations should be taken for the cases of liver traumatic rupture.
