纤维乳管镜在乳头溢液疾病诊治中的应用价值(附72例报告)

目的:通过纤维乳管镜检查,直接反映病变情况,同时对疾病进行诊断和治疗。方法:对2006年6月-2007年2月72例病人进行回顾性分析。全组病人术前均行乳腺导管鏡、乳头溢液细胞学检查、彩超检查。发现并确定病灶位置,记录病变导管的形态特征,对可疑病例定位切除送病解确诊。结果:溢液涂片检查找到癌细胞和可疑癌细胞6例(6/72),与术后病理相符。B超检查提示,导管内肿瘤16例(16/72),导管扩张症17例(17/72)。乳腺导管鏡提示导管内肿瘤12例(12/72)。其余21例未发现异常。乳管镜检对乳腺癌诊断的灵敏度为93.4%,特异度为98.5%,远高于乳腺导管造影。乳管镜检查结果与病理检查结果诊断符合率97.5%。结论:乳管镜有助于观察乳管内病变情况,从而有效确定乳头溢液疾病的病因及病变部位,诊断病变性质,提高乳腺癌的早期发现率。
