门诊药房用药差错原因分析及应对

目的防范用药差错的发生,确保患者用药安全有效。方法总结分析我院门诊药房常见用药差错的类型及其原因,提出应对措施。结果与结论通过有效的药品管理制度,规范可行的调剂操作规程,提高相关人员工作责任心及人文关怀等措施全方位提高药学服务质量,有效预防和减少用药差错的发生。

Analysis of medication error in outpatient pharmacy and countermeasures

Objective To prevent the occurrence of medication errors, and ensure patient＇s safety and effective use of medicine. Methods To analyze outpatient pharmacy common types of medication errors and their causes in our hospital, and propose countermeasures. Results and Conclusion Through effective drug management system, standardized and fea- sible dispensing operation procedures , enhancing related personnel the sense of responsibility and humanistic care and other measures, to improve pharmacy service quality, and effectively prevent and reduce the incidence of medication errors.
