腔内射频消融优化参数设置的离体猪肝实验研究

摘    要:目的采用离体正常猪肝组织进行腔内射频消融实验,探讨在不同功率与消融时间设置下消融导管产生的消融灶大小。方法采用新型消融导管在正常猪肝上进行射频消融实验,采用消融模式A(固定功率下持续消融至自动停止)进行射频消融,探讨在不同功率输出下最大消融时间及消融范围大小;采用消融模式B(特定功率下射频至特定时间内人为停止)进行射频消融,探讨不同参数设置下消融灶特性。结果采用消融模式A进行消融时.特定功率下均存在一定极限消融时间,功率越小,极限消融时间越长,其消融深度及范围越大。采用消融模式B进行消融时,在20W或以内的功率输出时,能产生有效消融灶,在此功率范围内,消融时间越长,其消融深度及范围越大。结论腔内射频消融功率输出应在20W以内,在此范围内消融时间越长。产生的消融深度及范围越大。

Abstract:Objective To explore the features of endoscopic radiofrequency ablation in normal ex-swine livers under different parameters setting which included radiofrequency energy output power and ablative time. Methods A new developed catheter with two metal electrodes ring embedded in the dismal tip was use for radiofrequency ablation in ex-swine livers. Mode A (ablation was performed under certain radiofrequency output power until automatically stopped by system) and Mode B (ablation was performed under different output power with different time) were respectively applied for the ablation. The maximum ablation time under Mode A and features of the ablation zone under both modes were recorded for analysis. Results Under Mode A, different maximum ablation time was observed according to different power outputs. The lower power output was applied, the longer maximum ablative time with larger ablative zone was achieved. Under Mode B, effective ablative zone was observed only with set power output no larger than 20 W. Under certain power output within 20 W, the longer ablation time within maximum time recorded by Mode A was applied, the larger ablative zone was achieved. Conclusions Endo-radiofrequency ablation should be applied with power output no larger than 20 W. Within 20 W, longer ablative time can lead to larger ablative zone.
