口腔医院突发医疗紧急情况应急能力的探讨

目的分析口腔医院出现突发医疗紧急情况的种类和原因，探讨提高口腔专科医院应对医疗紧急情况的应急处置能力和管理水平。方法统计南京大学医学院附属口腔医院近5年住院患者出现医疗紧急情况抢救报表；以及对54名口腔门诊医师进行突发医疗紧急情况问卷调查和急救技能考核。结果住院病人出现医疗紧急情况抢救者31例，在院抢救成功率93．5％；约30％口腔门诊医生遇到过医疗紧急情况，晕厥、轻度过敏反应最为常见，其余为心动过速／心率失常、心绞痛、体位性低血压，癫痫小发作，支气管痉挛等；约80％的被调查的口腔医师普遍存在急救知识和急救技能欠缺。结论口腔医院应重视突发医疗紧急情况，制定切实可行的各种应急预案，加强医护人员急救实践技能训练和考核，提高口腔医院的综合医疗能力。

Discussions of Medical Emergency Management in Stomatological Hospitals

Objective： To analyze the types and causes of medical emergencies in stomatological hospital. Methods： The medical emergencies occurred from 2008 to 2012 in Oral and Maxillofacial Surgery Department in the Stomatological Hospital of Nanjing University Medical School were analyzed; Survey on 54 dentists were performed for medical emergency treatment by question- naires, and all who were investigated had been trained and checked. Results： 31 in -patients had a medical emergency rescue in Oral and Maxillofacial Surgery ~ the survival rate was 93.5%. Approximately 30% of dentists had at least one patient with a medical emergency. About 30% of the dentists thoroughly understood the emergency medicine knowledge and skills . Conclu- sion： It is necessary to improve the awareness of emergency prevention and make the emergency preplan and strengthen the training on ability of emergency treatment among dentists.
