超关节外固定支架固定结合有限内固定治疗严重胫骨Pilon骨折疗效分析

目的探讨超关节外固定支架结合有限内固定治疗严重Pilon骨折临床疗效。方法对31例Ⅲ型Pilon骨折患者采用超关节外固定支架固定和有限内固定治疗，对其治疗效果进行分析总结。结果所有患者均获得随访，随访8个月--2年，平均14个月，无伤口感染，无神经、血管损伤等并发症，1例患者出现钉道感染，1例患者术后2个月出现骨折成角移位。骨折均愈合，平均愈合时间为4个月。踝关节功能评分：优12例，良9例，可7例，差3例。结论超关节外固定支架固定结合有限内固定治疗严重Pilon骨折手术创伤小、时间短，可避免伤口感染和骨不连的发生；踝关节可早期活动，避免了关节僵硬。

External fixation and limited internal fixation in treating severe tibial pilon fractures

Objective To investigate the clinical effects of external fixation and limited internal fixation in treating severe tibial pilon fractures.Methods 31 cases with severe pilon fractures were treated with external fixation and limited internal fixation with minimally invasive technology.Results All patients were followed up for 8~24 months.All fractures were healed.No vascular and nervous complications occurred.Tract infection occurred in one patient and angulation displacement occurred in one patient two months after surgery.Conclusion The treatment of tibial pilon fracture with external fixation and limited internal fixation is an effective method.It has the virtues of small incision,little damage of blood supply of fracture site and rigid fixation.It can prevent the stiffness of joint by early motion.
