腹腔镜联合药物治疗宫角妊娠及输卵管间质部妊娠的探讨

目的 探讨腹腔镜联合药物治疗宫角妊娠及输卵管间质部妊娠的价值。方法 将22例宫角妊娠、36例输卵管间质部妊娠病例分为3组（剖腹组、腹腔镜手术组、腹腔镜联合药物组），对其治疗结果，优越性及生殖结局进行比较。结果 孕50d后超声与腹腔镜检查的诊断率达100％，腹腔镜联合药物治疗成功率100％，达到剖腹手术的效果，而且术中出血最少，损伤最小，手术时间最短，术后病率最低，术后恢复最快，术后生殖功能恢复完全达到剖腹手术的效果。结论 腹腔镜联合药物治疗能达到微创效优的效果，值得临床推广。

Determination of laparoscopy combined with medical therapy in uterine horn and interstitial tubal ectopic pregnancy

Objective To determine the value of laparoscopy combined with medical therapy in uterine horn and interstitial uterine tubal ectopic pregnancy. Methods Twenty two cases of uterine horn pregnancies and 36cases of interstitial tubal gestations were treated with laparoscopy combined with medical therapy compared with laparotomy alone for evaluating the clinical effect superiority and the reproductive rate. Results After 50 days gestation, the laparoscopy and ultrasonography were used for delecting the clinical diagnosis. The consistency rate was 100%. The medical treatment combined with laparoscopy showed the sucsesful rate being 100%, the reproduction rates of the combined therapy group was the same as that in the laparotomy group. However, the bleeding amount and injury were significantly less, the operation time was much shortest, the mortality was lowest and the postoperation recorery earliest. Conclusion The laparoscopy combined with medical therapy is the least injury and the highest clinical effectiveness in the treatment of uterine horn and interstitial uterine tubal ectopic pregnancy, it might be popularized to clinical practice.
