CA125、AFP、CEA、CA19-9检测诊断卵巢癌的临床应用价值

目的　探讨糖链抗原 (CA12 5 )、甲胎蛋白 (AFP)、癌胚抗原 (CEA)、糖链抗原 (CA19 9)诊断卵巢癌的价值。方法　采集 2 96例住院患者的血清 ,其中卵巢癌 4 7例 ,卵巢良性病变 2 4 9例 ,用化学发光法检测其血清CA12 5、AFP、CEA、CA19 9值。结果　卵巢癌组患者血清CA12 5的阳性率 (6 3 83% ) ,明显高于良性卵巢病变组(P <0 0 1) ;浆液性卵巢癌CA12 5的阳性率 (88 89% ) ,明显高于黏液性卵巢癌的阳性率 16 6 7% (P <0 0 5 )。早期卵巢癌CA12 5、AFP、CA19 9、CEA联合检测阳性率较CA12 5单项检测的阳性率明显升高 (P <0 0 5 ) ;卵巢癌第一次手术切除瘤体 <10cm组与 >10cm组的CA12 5检测值差异无显著性 (P >0 0 5 )。结论　CA12 5在临床诊断卵巢癌中具有重要价值 ,尤以诊断浆液性卵巢癌为突出 ;CA12 5、AFP、CA19 9、CEA联合检测有助于提高早期卵巢癌的检出率 ;CA12 5检测值与卵巢癌第一次手术切除瘤体的大小无关。

Clinic evaluation of serum CA125,AFP,CEA,CA19-9 in diagnosis of ovarian cancer

Objective To evalute the diagnostic validity of serum CA125,AFP,CEA,CA19-9 in ovarian cancer.Metheds The serum CA125,AFP,CEA,CA19-9 were determinated by automated chemiluminescence assay in all 296 patients(including 47 cases of ovarian cancer,249 cases of ovarian benign diseases).Results The CA125 positive rates were higher significantly in ovarian cancer(63.85%)than in ovarian benign diseases(P<0.01).The CA125 positive rates were higher significantly in serous ovarian cancer 88.89% than in mucinous ovarian cancer 16.67%(P<0.05).In early stage of ovarian cancer,the combined detecting positive rates of CA125,AFP,CEA,CA19-9 were higher than CA125 detecting monomial(P<0.05).The serum CA125 were no difference in cancer size of first operation between the ovarian cancer dia >10 cm and the ovarian cancer dia <10 cm.Conclusions CA125 detecting has important diagnostic value for ovanrian cancer special for serous ovarian cancer;combined detecting of CA125,AFP,CEA,CA19-9 is helpful for early stage ovarian cancer diagnosis;serum CA125 level is no relation to the cancer size of first operation.
