肺曲霉菌病的螺旋CT诊断

目的通过分析肺曲霉菌病（pulmonary aspergillosis，PA）的影像特征，增强对肺曲菌球病的认识。方法对12例经病理活检证实的肺曲霉菌病进行回顾性分析。结果本研究中经病理证实为PA的螺旋CT主要表现包括渗出性病变（7/12）、薄壁空洞形成（7/12）伴“球中球”征象（空洞内可移动肿块及半月形空气征）（4/12）、多发结节（蜂窝状改变中见小团块密影）（5/12）、肿块样浸润灶伴“晕轮征”（软组织肿块，边缘可见“晕轮征”）（3/12）。结论肺曲菌病的影像学表现多种多样，但认识其在不同时期具有的相应某些特点的征象，有利于临床早期正确诊断并进行治疗。

Diagnosis of pulmonary aspergillosis by spiral computed tomograph

Objective To enhance the recognition and improve the diagnosis of pulmonary aspergillosis by studying the image features of pulmonary aspergillosis.Methods Twelve cases of lung aspergillosis confirmed by aspiration biopsy and bronchoscopy biopsy were summarized and analyzed.Results The dominating manifestations were as following:movable masses and semilunar air sign in the cavities,dense shadow of small masses within the alveolate structure,soft tissue masses with halo sign on the margin as shown by exudative process(7/12),thin wall cavity(7/12)accompanying ball in ball sign(4/12),multi-nodules(5/12),mass like infiltrating and halo sign(3/12).Conclusion There are multiple findings in pulmonary aspergillosis image,and some special appearances at different phases are valuable to make earlier diagnosis and therapy.
