不同剂量布地奈德联合异丙托溴铵雾化吸入治疗慢性阻塞性肺疾病急性加重期疗效观察

目的:观察不同剂量布地奈德联合异丙托溴铵雾化吸入治疗慢性阻塞性肺疾病急性加重期(AECOPD)的疗效。方法:将本院呼吸科住院部收治87例AECOPD患者随机分成两组,布地奈德高剂量组43例和低剂量组44例,两组常规治疗,同时加用布地奈德联合异丙托溴铵雾化吸入,观察治疗后两组患者动脉血气、肺功能、疗效、住院时间及出现不良反应。结果:两组治疗7 d后比较,临床症状评分,PO2及PCO2均有明显改善,治疗前后比较,P<0.05。高剂量组疗效优于低剂量组,治疗后FEV1与FEV1/FVC明显改善,住院时间显著缩短(P<0.05)。结论:布地奈德混悬液和异丙托溴铵联合雾化吸入具有较强的快速抗炎作用和缓解急性发作的疗效,提高布地奈德的剂量在一定程度上能增加疗效,但不良反应没有随之增加。

Clinical observation on effect of unequal Budesonide combined with ipratropium bromide in treatment of chronic obstructive pulmonary disease

Objective：To study the effect of unequal budesonide combined with ipratropium bromide in treatment of patients with chronic obstructive pulmonary disease. Methods： 87 patients with COPD were ran- domly divided into high dosage group 43 cases and low dosage group 44 cases, then after the treatment com- pared and analyzed the indicators blood gas, lung function, the clinical efficacy, the time in hospital and ad- verse reactions between patients of these two groups. Results： the effect is sure in these two groups, their differ- ence between these two groups was statistically significant （P 〈 0. 05）, adverse reactions are not increase in high dosage group. Conclusion： Budesonide joint with ipratropium bromide was an effective choice for treatment of patients with COPD, and to increase Budesonide dosage can raise the clinical efficacy and at the same time adverse reactions have not increase.
