| 摘 要： | 目的:为建立新型社区药学服务模式提供可行性依据。方法:基于本院下设社区医疗服务站的药学服务工作情况,以接诊患者的时间先后为序,从接诊前、就诊中、用药后3方面进行调查分析。结果与结论:该模式可通过接诊前的药学宣传、用药咨询,就诊中的完善患者健康资料、用药过程监测,用药后的药患之间互动、数据资料分析等方法建立;所建立的社区药学服务模式可行性强,效果良好。

| Abstract: | OBJECTIVE： To provide feasible basis for establishment of new model of pharmaceutical care in community. METHODS： Pharmaceutical care in time sequence of before or during visiting hospital, after medication were surveyed and analyzed, according to the pharmacy practice of the community medical service stations which belonged to our hospital. RESULTS ＆ CONCLUSIONS： New model was established through pharmaceutical propaganda, medication counseling, consummate patients＇ information, monitoring of medication process, communication of pharmacists and patients, data analysis, etc. The established model of pharmaceutical care in community is feasible and of high performance. |
