国产奥曲肽治疗肝硬化食管静脉曲张破裂出血疗效观察与护理

目的探讨国产与进口奥曲肽治疗肝硬化食管静脉曲张破裂出血的疗效及护理方法。方法将63例肝硬化食管静脉曲张破裂出血患者按给予国产奥曲肽和进口奥曲肽治疗分成观察组（32例）和对照组（31例），并给予心理护理、用药护理和出血护理等。观察奥曲肽治疗后24h内、25—72h出血停止情况及出血停止1周内再出血情况及副作用，并与进口奥曲肽进行疗效对比。结果观察组和对照组治疗后总有效率分别为84．4％（27／32）、87．1％（27／31），差异无统计学意义（P〉0．05）。出血停止1周内再出血率观察组为3．1％（1／32），对照组为3．2％（1／31）差异无统计学意义（P〉0．05）。2组均未发现明显副作用。结论国产奥曲肽在治疗肝硬化门静脉高压食管静脉曲张破裂出血方面与进口奥曲肽疗效相当，无明显副作用，且更为经济。精心的护理对抢救肝硬化食管静脉曲张破裂出血患者有重要意义。

Effect of domestic octreotide and nursing on esophagus varicosity breakage and hemorrhage

Objective To discuss the curative effect of octreotide and the nursing methods in the treatment of breakage and hemorrhage of esophagus varicosity. Methods All 63 patients with esophagus varicosity breakage and hemorrhage were divided into observation group and control group according to domestic and import octreotide. The patients in observation group were administered with domestic octreotide, while the patients in control group were administered with imported octreotide. All patients underwent mental nursing. The conditions of stopping bleeding in 72 h and bleeding again after stopping bleeding for one week, and side effect were observed and compared in observation group and control group. Results The total effective rates in observation group and control group were 84.4% （27/32）, 87.1% （27/31）, respectively, and there were not significant difference（P 〉 0.05 ）. The rebleeding rate in observation group was 3. 1% （1/32）, which was not higher than that in control group （3.2%）（1/31）] （P 〉 0.05）. No serious side effects happened. Conclusions In the treatment of hver cirrhosis, esophagus varicosity, breakage and hemorrhage, domestic octreotide, with no obvious side effect, has almost the same curative effect as foreign octreotide and is more economical. For patients suffering from liver cirrhosis and esophagus varicosity, breakage and hemorrhage, careful nursing has vital significance.
