某三级甲等综合性医院感染现患率及抗菌药物应用调查研究

摘    要:目的:了解某三级甲等综合性医院各科的感染现患率,并进行感染危险因素、抗菌药物应用等监测。方法:采用床旁调查和查阅住院病历相结合的方法,由该院各科室经过培训的感控医师和护士一起,对该院2008年11月27日901例住院患者的医院感染日现患率进行调查。结果:该院住院患者的医院感染日现患率为5.11%;呼吸道是最常见的感染部位,其次为泌尿道、皮肤和软组织、腹部及消化道、手术部位;医院感染日现患率最高的科室为综合重症监护室,达66.67%;抗菌药物应用率为65.49%。结论:抗菌药物合理应用监管仍是医院感控工作的重点和难点,需对医院感染日现患率较高的重点科室进行目标性监测。

Abstract:OBJECTIVE: To investigate the point prevalence rate of nosocomial infection in a class three grade A hospital and to monitor nosocomial infection and utilization of antibiotics. METHODS: The daily point prevalence rate of nosocomial infection in 901 inpatients in Nov. 27th of 2008 was investigated by trained doctors and nurses, using bedside investigation and medical record review. RESULTS: The daily point prevalence rate of nosocomial infection inpatients was 5.11%. The most common infection site was respiratory tract, followed by urinary tract, skin and soft tissue, gastrointestinal tract and surgical sites. The daily point prevalence rate of nosocomial infection in ICU was higher than other departments (66.67%). The utilization rate of antibiotics reached 65.49%. CONCLUSION: Antibiotic application monitoring is still the key to nosocomial infection control. The department with high daily point prevalence rate of nosocomial infection should be investigated with prospective methods.
