舒芬太尼与芬太尼用于剖宫产术后镇痛的对比研究

目的比较舒芬太尼与芬太尼用于剖宫产术后镇痛的效果及安全性。方法将河南省商丘市第一人民医院2011年1—12月行自控静脉镇痛的90例剖宫产患者完全随机分成观察组和对照组，各45例。对照组给予芬太尼静脉自控镇痛；观察组则给予舒芬太尼静脉自控镇痛。比较2组患者术后4、8、12、24、48h的疼痛视觉模拟评分（vAs），镇静过度、皮肤瘙痒、恶心呕吐、呼吸抑制、通气延长等不良反应的发生情况。结果观察组患者各时段的术后疼痛VAS评分均优于对照组（P〈0．05）；观察组的不良反应总发生率33．3％（15／45）]低于对照组66．7％（30／45）]，组间差异有统计学意义（P〈0．05）。结论舒芬太尼应用于剖宫产术后自控静脉镇痛优于芬太尼。

Clinical value of sufentanil and fentanyl agonist for analgesia after cesarean section

Objective To explore the clinical value of sulentanil and ientanyl agomst for analgesia alter cesarean section. Methods Totally 90 patients with patient-controlled intravenous analgesia（PCIA） from January 2011 to December 2011 were randomly divided into the observation group and the control group. The control group （45 cases）was given PCIA with fentanyl, while the observation group （45 cases） was given PCIA with sufentanil. Visual analogue scale（VAS） at 4, 8, 12, 24, 48 h after the operation, the occurrence of adverse drug reactions, such as pain, pruritus, nausea and vomiting, respiratory depression, ventilation prolongation were compared between the two groups. Results The observation group in 4 h, 8 h, 12 h, 24 h, 48 h had higher VAS scores than the control group （P 〈0.05） ; the total adverse reactions rate was 33.3% in the observation group and that in the control group was 66.7% （ P 〈 0.05 ）. Conclusion There is a certain value of sufentanil for PCIA after cesarean section.
