鼻内窥镜下治疗复发性泪囊炎临床分析

目的探讨泪囊鼻腔吻合术后复发性泪囊炎复发原因及鼻内窥镜下治疗复发性泪囊炎的疗效。方法回顾性分析鼻内窥镜行泪囊鼻腔吻合口再造术的复发性泪囊炎患者32例（34只眼），术前行鼻腔检查及泪囊碘油造影并分析复发原因，术后随访10～12个月。结果分析术后复发原因，为泪囊鼻腔吻合口闭锁，手术骨窗过小，泪囊萎缩，泪总管阻塞，合并鼻炎、鼻窦炎，鼻中隔偏曲，鼻甲肥大。32例经鼻内窥镜下治疗患者，治愈23眼，好转8眼，无效3眼，总有效率为91．2％。结论鼻内窥镜下泪囊鼻腔吻合口再造术操作简单，快捷，治愈率高，是治疗术后复发性泪囊炎的理想办法。

Clinical analysis on treating recurrent dacryocystitis by nasal endoscope

Objective To investigate the reason of recurrent dacryocystitis and the therapeutic effect of dacryocystorhinostomy under nasal endoscopy in treating recurrent dacryocystitis Methods The clinical data of 32 cases（34eyes） with recurrent dacryocystitis who accepted anastomotic stoma reconstructive operation were analyzed retrospectively.Nasal cavity examination and dacryocyst lipiodol opacification were executive before operation to investigate the reason of recur, and the patients were followed up for 10 to 12 months. Result After investigating the reason of recur, 30 cases were found anastomotic stoma occlusion, 3 cases found small bone window, 1 case found dacryocyst atrophy, 1 case found lacrimal ductile occlusion, 11 cases combined with nasal sinusitis and rhinitis, 7 cases combined with deflection of nasal septum, 1 cases combined with hypertrophy of nasal turbinates .At last, 23 eyes recoverd, 8 eyes improved, 3 eyes unimproved, total effective rate was 91.2%.Conclusion Anastomotic stoma reconstructive operation was an ideal method for recurrent dacryocystitis because it is shortcut, effective and easy to learn.
