全髋关节置换术后髋关节后脱位的分析与处理

目的探讨后外侧入路人工全髋关节置换术后早期出现髋关节后脱位的原因与防治措施。方法笔者对163例采取后外侧入路人工全髋关节置换术的临床资料及随访进行回顾性分析。结果平均随访5月~9年,发现共有5例患者早期出现髋关节后脱位。结论手术的方式、术后护理和早期康复训练不当,假体位置不良,髋关节周围软组织松弛,肌力不平衡,是本组病例发生早期出现髋关节后脱位的原因。一旦发生早期脱位,应及早复位,对于反复出现的脱位,明确病因,积极手术治疗。

Analysis and Treatment of Early Posterior Dislocation after Total Hip Replacement by Posterolateral Approach

Objective To investigate the cause and prophylaxis of the early posterior dislocation after total hip replacement （THR） by posterolateral approach. Methods 163 THR cases by posterolateral approach carried out by our department were followed up and analyzed. Results The follow-up of 5 months to 9 years showed 5 eases of early posterior dislocation. Conclusions The causes of the early posterior dislocation after TRP were related to approach of the operation, inappropriate rehabilitation training, malposition of the prosthesis, and surrounding soft tissue relaxation of the hip and muscular imbalance. Reduction should be in time when the dislocation occurred. The causes should be carefully analyzed for the recnn＇ent cases and aggressive operation should be considered.
