顺行腓肠神经岛状皮瓣修复膝前软组织缺损的临床研究（附12例报告）

目的探讨用顺行腓肠神经岛状皮瓣修复膝前软组织缺损的临床效果。方法2006年1月至2009年4月，应用顺行腓肠神经岛状皮瓣修复膝前软组织缺损12例（男10例，女2例），创面都有深部组织外露，切取皮瓣面积8cm×5cm～15cm×10cm，蒂长8～15cm，蒂旋转角度大于80°，经皮下隧道转位皮瓣。结果本组12例获得3～18个月随访，平均随访12个月，12例皮瓣全部成活，皮瓣有触痛觉，足跟外侧及足背外侧有皮肤感觉异常，但不影响生活、工作。结论该皮瓣是修复膝前软组织缺损的一种实用的好方法。皮瓣成活率高、手术操作简便、组织损伤小、皮瓣质地及外观好。

Clinical study of the use of forward flow sural nerve island flaps in reconstruction of soft tissue defect in front knee:Report of 12 cases

Objective To investigate the clinical effects of forward flow sural nerve island flaps in reconstruction of soft tissue defect in front knee. Methods From January 2006 to April 2009, 12 eases of soft tissue defect in front knee were being repaired by using forward flow sural nerve island flaps. Among the patients there were ten males and two females, all wounds with deeply exposed tissue. The area of the selected flaps varied from 8 crux 5 cm to 15 cmX 10 cm, with the length of fascia ranging from 8 cm to 15 cm and the rotation degree more than 80°. The flaps were twisted through subcutaneously tunnel. Resuits The 12 cases in this group received follow ups from three to eight months, averaging 12 months. All the 12 flaps survived. The sense of haphalgesia could be felt in the flaps. And there was an abnormal sensation in the lateral part of the heel and the lateral part of the foot, but it did not affect life and work. Conclusion The application of this kind of flap is a practical and satisfactory method to reconstruct soft tissue defect in front knee, with high survival rate of flaps, simple operation procedures, small tissue injury, and good flap texture and appearance.
