摘    要:目的:探讨肺表面活性物质(pulmonary surfactant,PS)不同应用方式对家兔急性肺损伤气体交换的影响。方法:18只健康新西兰大白兔经盐水反复灌洗后致PaO2/FiO2<200mmHg,随机分为3组,每组6只,PS(100mg/kg)采用注入法(T1组)和肺灌洗法(T2组),对照组(T3组)采用同等量生理盐水滴入。观察治疗后PaO2/FiO2、PaCO2和气道吸气峰压(PIP)的变化。结果:T1、T2组治疗后各时间段PaO2/FiO2均较治疗前明显改善,T2组较T1更为显著(P<0.05);T2组气道峰压(PIP)较治疗前明显下降,并与其它各组相比差异有显著性(P<0.05)。结论:ARDS早期采用外源性PS替代治疗可以明显改善肺氧合,肺灌注方法较注入法更优。

Abstract:Objective: To investigate the effect of different administrations of pulmonary surfactant on pulmonary gas exchange after acute lung injury in rabbits. Methods:The rabbits with PaO2/FiO2 less than 200mmHg after lung lavage by normal saline were randomly divided into three groups, with 6 in each group. The pulmonary surfactant( 100mg/kg)was given by instillation(T1 group)and by pulmonary lavage(T2 group).The same volume of saline was used to substitute the pulmonary surfactant in T3 group.Results: PaO2/FiO2 dramatically improved in group T1 and T2(P < 0.05) . The PIP decreased markedly(P < 0.05)soon after the lungs were lavaged by pulmonary surfactant in group T2. Conclusion: Lavage administration of pulmonary surfactant seems to be a promising method in the treatment of acute lung injury.
