儿童重症监护室呼吸机相关肺炎病原学分析

摘    要:目的:探讨儿童重症监护室(PICU)呼吸机相关肺炎(VAP)的临床病原学特点。方法:对156例PICU机械通气患儿的临床资料、病原菌构成和药敏试验结果进行分析。结果:156例机械通气患儿中,66例发生VAP,发生率为42.4%,早发性VAP病原菌以革兰阳性菌为主,晚发性VAP以革兰阴性菌为主,耐药性较高。结论:明确VAP的发病因素和致病菌,有利于合理使用抗生素,预防VAP。

Abstract:Objective: To analyze the clinical and etiological characteristics and bactcrial susceptibility of children with ventilator-associated pneumonia in PICU.Methods: The mechanically ventilated patients in PICU of Xi‘an children’s hospital were studied retrospectivly in regard to clinical data ,pathogen analysis and bacterial susceptibility.Results: Among the 156 ventilated childen,66 cases of VAP were diagnosed with the incidence rate of 42% .The major pathogens of early onset VAP were identified as Gram-positive bacteria;Gram-negative pathogens predominated in late onset VAP.Conclusion: Early detection of the VAP risk factors and pathogens will facilitate antibiotic therapy to prevent and treat VAP.
