经颅多普勒对高血脂患者脑动脉硬化的研究

目的分析高血脂患者脑血流动力学的改变,探讨经颅多普勒对高血脂患者脑血管疾病的临床应用价值。方法应用经颅多普勒超声(TCD)检测120例高血脂患者脑血流动力学的改变,并与100例健康者(对照组)进行比较。结果高血脂组脑动脉硬化者86例,占71.7%,与对照组比较,差异有极显著性(P<0.001);脑动脉硬化伴狭窄者16例,占13.3%,与对照组比较,差异有显著性(P=0.013);血管狭窄受损血管主要为颈内动脉系,并以大脑中动脉多见;血流速度减慢受累动脉以椎-基底动脉系为主,多见于单侧椎动脉。结论TCD检查显示高血脂患者脑血流动力学有明显异常,脑动脉硬化是高血脂患者的常见并发症,提示TCD可作为高血脂患者的常规检测手段,对预防高血脂患者脑血管疾病的发生有一定的临床价值。

A Clinical Study of Transcranial Doppler in Diagnosing Cerebroarteriosclerosis for Patients with Hyperlipemia

Objective To evaluate the clinical efficacy of TCD in diagnosing cerebrovascular diseases for patients with hyperlipemia.Methods 120 patients with hyperlipemia and 100 health controls had been examined by TCD,the data of both groups were statistically compared.Results In hyperlipemia group,86 patients (71.7%) had been diagnosed as cerebroarteriosclerosis and 16 patients (13.3%) as cerebrovascular stricture which were mainly located at internal jugular vein,especially at middle cerebral arteries.Lower blood flow velocity was also observed in unilateral vertebral and basilar arteries.Comparing with the health control group,the differences were significant (P<0.001,P=0.013).Conclusions TCD shows there are significant abnormalities in patients with hyperlipemia.Cerebroarteriosclerosis is a common complication for patients with hyperlipemia.TCD could be used as a routine test for hyperlipemia patients to predict the risk of cerebrovascular diseases.
