脊髓半切后胫后神经体感诱发电位与后肢运动功能的相关性研究

摘    要:目的:研究成年猴脊髓半切后胫后神经体感诱发电位(T-SEPs)与后肢运动功能的关系。方法:雄性成年恒河猴2只,半切右侧脊髓L1节段,5个月后分别于左侧和右侧T11节段行第二次脊髓半切。分别于两次手术前后不同时间点检测后肢T-SEPs及肌力的变化。结果:首次脊髓半切致动物右后肢完全瘫痪,右侧T-SEPs消失,左侧的潜伏期(PL)延长、波幅降低,随瘫肢肌力的恢复,右侧T-SEPs重新出现与左侧T-SEPs同时改善。左侧二次脊髓半切后双后肢完全瘫痪,双侧T-SEPs消失;右侧二次半切后仅出现双后肢肌力轻度减退,伴右侧T-SEPs再度消失,左侧PL延长,波幅度低,统计分析示PL、峰峰值与肌力变化间存在高度相关性。结论:成年猴脊髓半切后T-SEPs变化与后肢运动功能密切相关,为脊髓损伤后后肢运动功能的一项间接、客观的检测指标。

Abstract:Objective To investigate the relation between tibia somatosensory evoked potentials (T SEPs) and locomotor function of paraplegic hindlimbs following spinal cord hemisection in adult rhesus monkeys. Methods Two adult rhesus monkeys (E1 and E2) received the first right spinal cord hemisection on L 1 and the second spinal cord hemisection on T 11 contralaterally (E1) and ipsilaterally (E2) at the initial lesion 5 months later. Changes of T SEPs and muscle strength of the hindlimbs were recorded and analyzed statistically at different time points before and after the first and the second operations. Results The first spinal cord hemisection on the right L 1 resulted in a complete paraplegia of the right hindlimb in both animals. Meanwhile, T SEPs on the right side disappeared and the potential latencies (PL) prolonged and amplitudes on the left side decreased. When muscle strength of the paraplegia hindlimb was increasing with time, the right T SEPs recurred and the left T-SEPs improved. Statistic analysis showed a high correlation between the changes of T SEPs and the locomotor function of the hindlimbs after spinal cord hemisection. The hindlimbs of E1 became paraplegic completely after the second hemisection on the left side at T 11 , and T SEPs of both sides disappeared. As for E2 animal, however, only mild paraplegia was found in both hind limbs following the second hemisection on the right side, with disappeared T SEPs on the right side and prolonged PL as well as decreased amplitudes on the left side. Conclusions T SEPs may provide a indirect and objective method for assessing locomotor function after spinal cord injury in adult monkeys.
