氯吡格雷对糖尿病肾病早期微量蛋白尿的治疗作用观察

目的探讨氯吡格雷对糖尿病肾病早期微量蛋白尿的干预作用。方法将90例伴有微量蛋白尿的2型糖尿病患者随机分为3组：空白对照组30例;氯吡格雷组30例及胰激肽原酶（TPK）对照组,治疗12周。观察患者治疗前后糖化血红蛋白（HbA1c）、总胆固醇（TC）、三酰甘油（TG）、低密度脂蛋白（LDL-C）、高密度脂蛋白（HDL-C）和尿清蛋白/肌酐比率（UACR）的变化。结果氯吡格雷组、胰激肽原酶对照组与空白对照组比较,UACR均明显下降,氯吡格雷组与胰激肽原酶对照组比较,尿清蛋白的变化差异无统计学意义。空白对照组治疗前后尿清蛋白无明显变化。各组治疗前后HbA1c均有显著变化,治疗后各组间比较差异无统计学意义,各组治疗前后TC、TG、HDL-C和LDL-C均无明显改变。结论氯吡格雷能显著降低糖尿病早期肾病微量蛋白尿,对糖尿病早期肾病有治疗作用。

Effect of Clopidogrel on patients with early diabetic nephropathy

Objective To investigate the effecacy of Clopidogrel in treating incipiet diabetic nephropathy in type 2 diabetic patients.MethodsNinty cases of type 2 diabetes with microalbuminura were randomized into three groups in each.the Clopidogrel-treated group was orally given Clopidogrel（75mg,qd） for 12 weeks.The TPK-treated group was orally given（120u,tid） for 12 weeks too.The control group was without relevant therapy for 12 weeks.Before and after the study the change of hemoglobin Alc（HbA1c）,total cholesterol（TC）,riglyceride（TG）,low density lipoprotein（LDL-C）,high density lipoprotein（HDL-C）,albumin-to-creatinine ratio（UACK）was investigated.ResultsIn Clopidogrel-treated group UACR was markedly decreased.The TPK-treated group UACR was strikingly decreased too,and did not show any significantly change in control group.HbA1c was obviously decreased,but there is no significant difference between 3 groups.And there were no statistically significant differences among the three groups in TC,TG,HDL-C and LDL-C.Conclusion Clopidogrel can decrease UACR efficiently in type 2diabetic patients with incipient diabetic nephropathy.
