FEV3/FVC在肺功能检查中临床意义的研究

目的 研究FEV3/FVC在肺功能检查中的临床意义.方法 将608人分为A、B、C三组,A组FEV1/FVC、FEV3/FVC正常（n=208）,B组仅FEV3/FVC下降（n=198）,C组FEV1/FVC下降（n=202）,观察各组肺功能各项指标的变化.结果 与A组比较,B组有较高的TLC（96.5±15.3％ vs 99.5±18.3％,P＜0.01）,RV （112.5±32.1％ vs 101.3±26.6％,P＜ 0.01）,RV/TLC（115.8±27％vs 101.3±31.3％,P＜0.01）.有较低的FEV1 （82.4±16.7％ vs 90.2±13.5％,P＜0.01）,IC（98.2±21.5％ vs 92.5±31.2％,P＜0.01）,DLCO （80.7±19.4％ vs 73.2±16.8％,P＜0.01）.B组的年纪则更偏大些（57.2±11.3 vs 61.2±12.5,P＜0.01）.B组与C组比较,C组有更高的TLC、RV、RV/TLC改变P＜0.01）,而FEV1、IC、DLCO的降低更明显,两组比较有显著差异（P＜0.01）.结论 FEV3/FVC应在肺功能中常规检查,FEV3/FVC的单独下降可能提示气道早期轻度功能障碍.

The Clinical Significance of FEV3/FVC in Pulmonary Function Tests

Objective To study the clinical significance of FEVflFVC in pulmonary function tests. Methods A total of 608 subjects were divided into three groups： 1） normal FEV1/FVC and FEV3/FVC （n=208）, 2） only a reduced FEV3/FVC （n=198）, 3） reduced FEVJFVC （n=202）. We observed the changes of lung function indexes. Results B group compared with A group had higher predicted TLC（96.5±15.3% vs 99.5±18.3%, P 〈 0.01）, RV（ 112.5±32.1% vs 101.3±26.6%, P 〈 0.01 ）, and RV/TLC（115.8±27% vs 101.3±31.3%, P 〈 0.01）. They had lower predicted FEV1 （82.4± 16.7% vs 90.2± 13.5%, P 〈 0.01）, IC （98.2±21.5% vs 92.5±31.2%, P〈 0.01）, DLCO （80.7±19.4% vs 73.2±16.8%, P〈 0.01）. B group were older（57.2±1l.3 vs 61.2±12.5, P 〈 0.01）. Compared to B group, C group had higher predicted TLC, RV, and RV/ TLC. Moreover, they had lower predicted FEV1, IC, DLCO. Conclusion The FEV3/FVC should be routine in lung function spirometry, An isolated reduced FEV3/FVC may suggest that early mild impairment of airway.
