摘    要:目的:探讨AECOPD合并肺栓塞(PE)的临床特点.方法:回顾性分析2012年1月至2014年1月确诊AECOPD合并PE 38例患者的临床资料.结果:AECOPD继发肺心病患者易合并PE(86.8%),最突出的症状是咳嗽咳痰(92.1%)及呼吸困难(76.3%),体征常见窦性心动过速(73.7%)、下肢不对称性肿胀(55.3%);动脉血气PaCO2下降或呈Ⅰ型呼吸衰竭表现;D-二聚体>0.5 mg/L患者占84.2%,超声心动图多伴肺动脉高压(占65.8%),下肢深静脉血栓形成患者占76.3%.结论:AECOPD合并PE有一定的临床特点,根据这些特点对诊断合并PE有一定临床意义.

The clinical analysis of 38 cases of acute exacerbation of chronic obstructive pulmonary disease combined with pulmonary embolism

Abstract:Objective: To investigate the clinical characteristics of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) combined with pulmonary embolism (PE). Methods: The clinical data of 38 cases of AECOPD combined with PE diagnosed from January,2012 to January, 2014 were analyzed retrospectively. Results: AECOPD complicated with corpulmonary was easily prone to combining with PE (86.8%). The most apparent symptom were cough and expectoration (92.1%) ,and dyspnea (76.3%). The common sign were the fast heart rate (73.7%) and asymmetric swelling of lower limbs(55.3%). Decrease of PaCO2 or type I respiratory failure appeared in blood gas analysis,D-dimer〉0.5mg/L accounted for 84.2 %, deep venous thrombosis accounted for 76.3%, hypertension of pulmonary artery were found in ultrasonic cardiogram (65.8%). Conclusion: There are some clinical characteristics in AECOPD combined with PE, which should be alert to the diagnosis of PE.
