监测危重患儿血清胆碱酯酶及白蛋白的临床意义

目的：探讨危重症患儿血清胆碱酯酶及白蛋白水平的变化及临床意义.方法：检测128例危重患儿及120例普通患儿的胆碱酯酶及白蛋白,将所有患儿进行危重病例评分,分成3组,比较3组间血清胆碱酯酶、白蛋白的差异,分析其与患儿预后的相关性.结果：危重患儿胆碱酯酶及白蛋白水平呈正相关（r=0.938,P＜0.01）.极危重组胆碱酯酶、白蛋白水平明显低于危重组及非危重组,差异有统计学意义（P＜0.01）,危重病例评分值越低,胆碱酯酶、白蛋白水平越低,预后越差,病死率越高.结论：胆碱酯酶、白蛋白水平与患儿危重病例评分是正相关,三者结合是判断患儿病情及预后较为简便、实用、敏感的指标.

Clinical significance of monitoring the serum cholinesterase and albumin of critically-ill children

Objective： To investigate the changes and clinical significance of serum cholinesterase （CHE） and albumin （ALB） of critically-ill children. Methods： CHE and ALB of 128 critically-ill children and 120 ordinary-ill children were detected for comparison and correlation analysis. The children patients were divided into 3 groups,then the differences of CHE and ALB were compared, and their correlations with ill children＇s prognoses were analyzed. Results： CHE and ALB of the critically-ill children were significantly and positively correlated （r= 0. 938, P〈0.01）. In the extremely critically ill group, the CHE and ALB levels were remarkably lower than those of critically ill group and non-critically-ill group, which were of statistical significances （P〈0. 01）. The lower score of the critically ill children had,the lower level of CHE and ALB,the worse prognosis and the higher death rate were for the children. Conclusion： CHE and ALB levels are obviously correlated with the scoring of critically-ill children. The combination of three has provided the simple, practical and sensitive indicators to determine the ill children＇s conditions and prognoses.
