自体外周血干细胞移植治疗股骨头无菌性坏死

目的 探讨髓芯减压＋同种异体骨植骨＋自体外周血干细胞（PBSC）移植治疗股骨头无菌性坏死（ANFH）的疗效.方法 对10例ANFH患者（16髋）行髓芯减压＋同种异体骨植骨＋自体PBSC移植治疗,比较术前与末次随访疼痛评分、Harris总评分及术前术后髋关节影像学资料.结果 术后患者疼痛症状均缓解.10例均获随访,时间10～24个月.X线片及CT提示股骨头坏死区明显减小或消失,关节间隙接近正常,髋关节功能明显改善.疼痛评分与Harris评分均明显高于术前（P〈0.05）.结论 应用髓芯减压＋同种异体骨植骨＋自体PBSC移植治疗ANFH,可明显减轻患者疼痛症状,恢复关节功能,延缓病情发展,且创伤小,操作简便,效果良好.

Autologous peripheral blood stem cell transplantation for the treatment of aseptic necrosis of femoral head

Objective To explore the efficacy of core decompression, allogeneic bone graft and autologous peripheral blood stem cell （PBSC） transplantation in the treatment of aseptic necrosis of the femoral head（ANFH）. Methods The 10 patients （ 16 hips） were treated by cord decompression, artificial bone and autologous peripheral blood stem cell transplantation. Preoperative and postoperative pain score, Harris hip score and radiographic data were recorded. Results Ten patients were followed up for 10 - 24 months. The area of necrosis in femoral head was reduced or disappeared, and joint space turned be normal by the X-ray and CT scan and and hip function were improved. After surgey, the pain score and Harris hip scores were significantly higher than that of preoperation（ P 〈 0. 05 ）. Conclusions The combination of core decompression, artificial bone and autologous PBSC transplantation for treatment of ANFH, can significantly reduce pain symptoms, restore joint function, and delay the development of osteonecrosis disease. This method for treatment of femoral head necrosis have advantages as tiny trauma, simple operation and nice curative effect.
