Prolift盆底重建术治疗重度盆腔器官脱垂的疗效评价

目的 评价应用Prolift全盆底重建术治疗重度盆腔器官脱垂的临床效果.方法 45例POP-Q分度为Ⅲ度和Ⅳ度的盆腔器官脱垂患者施行保留子宫的Prolift全盆底重建术,术后定期随访,以POP-Q分度为客观疗效评价指标,以对手术满意度的视觉模拟评分（VAS）为主观疗效评价指标.结果 手术时间（86.3±28.1） min,术中出血量（178.5±53.8） ml,术后平均住院（6.8±2.4）d,随诊6～18个月,治愈率为97.8%（44/45）.术后1个月、3个月、6个月、1年的POP-Q分度测量值与术前比较差异均有统计学意义（P＜0.05）,术后1个月、3个月、6个月、1年的VAS与术前比较差异均有统计学意义（P＜0.01）.结论 Prolift全盆底重建术安全、微创、有效、并发症少,术后生活质量明显改善,值得临床推广.

Efficacy evaluation of prolift pelvic floor reconstruction surgery for severe pelvic organ prolapse

Objective To evaluate the clinical efficacy of prolift pelvic floor reconstruction surgery in the treatment of severe pelvic organ prolapse.Methods 45 patients with pelvic organ prolapse whose pelvic organ prolapse quantitative（POP-Q）score was Ⅲ -Ⅳ underwent prolift pelvic floor reconstruction surgery with uterus reserved and the patients were followed up regularly after surgery.Then,the efficacy was assessed objectively according to POP-Q examination scores and the operation satisfaction was subjectively assessed according to visual analogue scale（VAS）. Results The average operation time was （86.3 ± 28.1） minutes.The average amount of blood loss during operation was （ 178.5± 53.8） ml.The average hospitalization time after operation was （ 6.8 ± 2.4） days.All the patients were followed up for 6-18 months and the cure rate was 97.8%（44/45）.The POP-Q scores of month 1,month 2,month 3, month 4 and one year after operation were significantly different from those before operation（ P 〈0.05）.The VAS of month 1 ,month 2,month 3,month 4 and one year after operation were also significantly different from those before operation（ P 〈0.01）.Conclusion Prolift pelvic floor reconstruction surgery is safe, effective, minimally invasive. It has few complications and can improve patients＇ quality of life after operation obviously.So it is worth clinical application.
