IL-8基因在胃癌及癌前状态中的表达

目的：探讨IL－8基因与癌前状态与胃癌发生发展的关系。方法：采用RT－PCR检测25例胃癌及癌旁正常粘膜中IL－8基因mRNA表达；免疫组化技术检测283例不同胃粘膜标本IL－8蛋白表达，其中包括29例正常胃粘膜、54例浅表性胃炎、33例胃溃疡、57例萎缩性胃炎、40例异型增生、70例胃癌。结果：1）胃癌组织中IL－8 mRNA及蛋白水平显著高于正常胃粘膜（P＜0．01）；2）正常胃粘膜、癌前状态与胃癌组织IL－8蛋白阳性表达率逐渐升高（P＜0．01），胃癌IL－8蛋白阳性表达率与癌前状态相比有显著性差异（P＜0．01）；3）浅表性胃炎、胃溃疡、异型增生、萎缩性胃炎及胃癌IL－8蛋白阳性表达率依次上升（29．6％、45．5％、57．5％、71．9％、88．6％），胃癌与萎缩性胃炎相比有显著性差异（P＜0．05）；胃癌与浅表性胃炎、胃溃疡、异型增生相比差异非常显著（P＜0．01）；萎缩性胃炎与异型增生中IL－8蛋白阳性表达率均高于浅表性胃炎（P＜0．01或P＜0．05）；萎缩性胃炎中IL－8蛋白阳性表达率高于异型增生。结论：IL－8基因在癌前状态及胃癌中高表达，IL－8基因表达增强与胃癌的发生发展及分化程度密切相关并与癌前状态的进展有关。IL－8在肿瘤中的作用机制以及临床应用前景有待于进一步研究。

The Expression of IL-8 gene in Precancerous Condition and Gastric Cancer

Objective:To investigate the relationship between IL-8gene and tumorigenesis,progression of precancerous condition and gastric cancer.Methods:The expression of IL-8mRNA was determined by RT-PCR in25cases of gastric cancer and corresponding non-cancerous mu-cosa.The expression of IL-8protein was determined by immunohistochemical method in283gastric specimens including29normal mucosa,54gastritis,33gastric ulcer,57atrophic gastritis,40dysplasia of gastric epithelium and70specimens of gastric cancer.Results:1)The levels of IL-8MRNA and protein expression in gastric cancer were significantly higher than that of corre-sponding non-cancerous mucosa(P<0.01);there were significant differences between gastric cancer and gastric ulcer and dysplasia of gastric epithelium(P<0.01).The positive rate of IL-8protein expression in dysplasia of gastric epithelium was higher than that of gastritis(P<0.01,P<0.05),and the positive rate of IL-8protein expression in atrophic gastritis was higher than that of dys-plasia of gastric epithelium.Conclusion:The high expression of IL-8gene in the precursor of gastric cancer and gastric cancer was first confirmed,and the increase of IL-8gene expression was related to the tumorigenesis,progression and the degree of tumor differentiation.Further studies will be necessary to determine the mechanisms of IL-8in the tumor and prospects of clinical implication.
