认知功能对恢复期缺血性脑卒中老年患者跌倒风险的影响

摘    要:目的探讨认知功能对恢复期缺血性脑卒中老年患者跌倒风险的影响。方法采用整群抽样法,运用蒙特利尔认知评估量表评估患者认知功能,Faster counting while walking测试法评估患者跌倒风险。将认知功能和跌倒风险纳入单因素及多因素Logistic回归模型筛选预测因子。结果跌倒高危风险组和低危风险组的认知总分、注意力、抽象能力和定向力得分差异有统计学意义。既往职业类型,认知总分,认知维度中的注意力、抽象能力和定向力对恢复期缺血性脑卒中老年患者跌倒风险具有预测作用。结论认知功能对恢复期缺血性脑卒中老年患者跌倒风险具有预测作用,关注认知因素对恢复期缺血性脑卒中老年人群跌倒的重要影响,可及时、动态地发现跌倒高危对象,以降低跌倒发生率。

Abstract:Objective To investigate the impact of cognitive function on fall risk of elderly patients with ischemic stroke in convalescence. Methods By cluster sampling, the Montreal Cognitive Assessment (MoCA) and the Faster Counting while Walking Test were used to measure cognitive function and fall risk respectively. Predictors of fall risk were screening out by logistic regression analysis. Results Scores of cognitive function, attention, abstract ability and orientation in high-risk group were significantly lower than those in low-risk group. Former occupations, cognitive function, attention, abstract ability and orientation finally entered the logistic regression prediction models. Conclusion Cognitive function can predict fall risk of elderly patients with ischemic stroke in convalescence. Risk of falls may be reduced by screening high risks and improving high risk patients ’ cognitive function timely.
