超声血管增强成像在椎动脉近端狭窄中的诊断价值

摘    要:目的评价超声血管增强成像(ClarifyvE)技术对椎动脉近端狭窄的诊断价值。方法以数字减影血管造影(DSA)或cT血管造影(CTA)为诊断标准,分析脉冲多普勒(Pw)和Oa~ifyVE诊断椎动脉近端不同程度狭窄的敏感性及特异性。结果67例椎动脉近端不同程度狭窄患者,DSA或CTA诊断椎动脉轻度狭窄4例,中度狭窄34例,重度狭窄29例。PW诊断椎动脉近段狭窄的符合率为91.0%;其诊断敏感性、特异性在轻度狭窄中分别为91.7%、98.5%,中度狭窄中分别为91.4%、79.4%,重度狭窄中分别为89.6%、94.1%。ClarifyVE诊断椎动脉近段狭窄的符合率为92.0%;其诊断敏感性、特异性在轻度狭窄中分别为80.3%、97.1%,中度狭窄中分别为94.1%、81.3%,重度狭窄中分别为93.1%、98.1%。结论ClarifyvE技术能够清晰显示斑块及残余管腔,可快速有效诊断椎动脉狭窄程度。

Abstract:Objective To evaluate the value of Clarify VE in the diagnosis of proximal vertebral artery stenosis. Methods Digital subtraction angiography (DSA) or CT angiography (CTA) was used as diagnostic criteria, 67 patients with proximal vertebral artery stenosis were analyzed using pulse Doppler waveforms (PW) and Clarify VE. Results There were 4 cases of mild proximal vertebral artery stenosis, 34 cases of moderate proximal vertebral artery stenosis and 29 cases of severe proximal vertebral artery stenosis. The coincidence rate in PW diagnosis proximal VA stenosis was 91.0%. The sensitivity and specificity in PW diagnosis of mild degree VA stenosis were 91.7% and 98.5%, respectively, in moderate degree VA stenosis, they were 91.4% and 79.4%,in severe degree VA stenosis,they were 89.6% and 94.1%. The coincidence rate in Clarify VE diagnosis proximal VA stenosis was 92.0%. The sensitivity and specificity in Clarify VE diagnosis of mild degree VA stenosis were 80.3% and 97.1%, respectively,in moderate degree VA stenosis,they were 94.1% and 81.3%,in severe degree VA stenosis, they were 93.1% and 98.1%. Conclusion Clarify VE technology can display distinctly plaque and residual lumen, which can rapidly and effectively diagnose the degree of vertebral artery stenosis.
