全身成像三维量化评估二尖瓣反流与实时三维超声心动图的对比研究

摘    要:目的探讨全身成像三维量化(G13DQ)法测量二尖瓣反流束体积的可行性和准确性。方法93例二尖瓣反流(MR)患者:偏心性MR组52例,中心性MR组41例。G13DQ法直接测量MR体积,以实时三维超声心动图(RT3DE)计算的MR体积作为参考标准。结果在偏心性MR组,两种方法相关性:r=0.91;一致性分析:G13DQ法较RT3DE法平均低估MR体积约10.6ml;在中心性MR组,两种方法相关性:r=0.96;一致性分析差异无统计学意义。结论在偏心性MR组,G13DQ法评估MR体积存在明显低估;在中心性MR组,G13DQ法与RT3DE法未见明显差异。

Abstract:Objective To evaluate feasibility and accuracy of mitral regurgitant volume (MRvol) by quantification of mitral regurgitation (MR) jet volume using general imaging 3-dimensional quantification (GI3DQ). Methods Ninety-three patients with MR were divided into central MR group (n=41) and eccentric MR group (n=52). Mitral regurgitation jet volume was measured using GI3DQ. The MRvol calculated by real-time 3-dimensional echocardiography (RT3DE) was used as reference standard. Results In eccentric MR group, MRvol measured by GI3DQ method with a lower underestimation (mean difference = -10.6 ml) showed good correlation with MRvol measured by RT3DE(r = 0.91 ). In central MR group, the correlation of MRvol measured by GI3DQ method and by RT3DE method was better (r = 0.96 ), but had no significant difference. Conclusion Quantification of central MRvol with GI3DQ was as accurate as RT3DE,but GI3DQ method underestimated eccentric MRvol.
