彩色多普勒超声心动图对先天性心脏病封堵术的疗效评价

目的　应用彩色多普勒超声心动图(彩超)评价先天性心脏病(先心病)封堵术的疗效。方法　3 9例继发孔型房间隔缺损( atrial septal defect,ASD) ,2 4例动脉导管未闭( patent ductus arteriousus,PDA)和11例膜部室间隔缺损( ventricular septal defect,VSD)。均采用Amplatzer封堵器进行治疗。经胸壁彩超获取图像。术前,术后,3 d及半年复查超声心动图,观察房、室间隔及大动脉水平有无分流,心脏大小变化。结果　3 9例ASD封堵成功3 4例,2 4例PDA均成功封堵,11例VSD封堵成功10例。封堵术后立即观察分流消失,术后3 d复查,有1例ASD患者存在少量残余分流,1例VSD患者主动脉瓣下少量返流。结论　彩超在先心病封堵术中对术前筛选患者,选择封堵器的大小,术后检测有无残余分流,对心脏血流动力学评价及疗效的观察起到了重要作用

Echocardiographic Evaluation of Transcatheter Closure of Congenital Heart Disease

Objective To evaluate the efficiency of transcatheter closure of congenital heart diseasse using echocardiography.Methods Of the 74 patients,there were 39 cases of secundum atrial septal defects (ASD),24 cases of patent ductus arteriosus (PDA),and 11 cases of ventricular septal defect (VSD).All of the patients underwent transcatheter closure using the Amplatzer occluder.The pictures were taken from transthoracic echocardiography(TTE).In all patients,echocardiography was performed before 3 days and half a year after operation,in order to meature the size of the hearts and to find whether there were shunts in atrial septum,ventricular septum and the large arteries.Results The successful transcatheter closure of ASD,PDA,and VSD were 34,24,10 cases respectively.Shunts dispeared immediately after the operation.Three days after operation,a slight shunt was detected in 1 case of ASD,and a little regurgitation was remained in the aortic valve of 1 VSD patient on ecdocardiography.Conclusions Echocardiography is of great usefulness in choosing patients,selecting the size of occluders,detecting whether there are residual shunts after operation and is important for evaluation of hemodynamic changes and curative effects.
