无创正压通气治疗慢性阻塞性肺疾病并呼吸衰竭51例临床观察

目的评价无创正压通气（NPPV）慢性阻塞性肺疾病的临床效果。方法 51慢性阻塞性肺疾病并2型呼吸衰竭患者采用德国CARAT呼吸机经口鼻正压通气,记录通气治疗前及通气治疗后2、4 h的血酸碱度（PH）、氧分压（PaO2）、二氧化碳分压（PaCO2）和血氧饱和度（SaO2）指标变化。结果无创正压通气后PaO2,SaO2均明显升高,PaCO2明显降低,PH明显改善,差异非常显著（P〈0.01）。结论无创正压通气治疗慢性阻塞性肺疾病并2型呼吸衰竭,提高PaO2和SaO2,降低PaCO2,纠正PH,疗效满意,简单实用,值得临床应用。

Clinical observation of non-invasion positive pressure ventilation(NPPV)therapy on chronic obstructive pulmonary disease associated with respiratory failure

Objective To evaluate the effect of NPPV in the treatment of COPD.Methods 51 patients with COPD and type Ⅱ RF were positively ventilated through oral-nasal way with CARA respiratory machine（German made）.The change of PH,PaO2,PaCO2 and SaO2 were recorded before and after ventilation at the time of 2 h and 4 h.Results PaO2 and SaO2 increased while PaCO2 reduced remarkably.PH improved with statistical significance（Pd0.01）.Conclusion NPPV therapy for COPD with type Ⅱ RF increases PaO2 and SaO2 while reduces PaCO2 and adjusts PH.
