经尿道前列腺电气化术后再入院的原因探讨

目的:探讨经尿道前列腺电气化术再入院原因及处理措施。方法与结果:对4例腺体残留或复发均施行经尿道电气化术治疗,术后尿路梗阻症状解除。5例尿道狭窄,其中2例经尿道瘢痕组织冷刀切开加电切术;2例单纯行经尿道狭窄冷刀切开,1例行尿道外口切开术,定期尿道扩张后能自行排尿。6例继发性出血再入院膀胱镜检冲洗凝血块,放置三腔尿管牵扯引压迫止血。2例前列腺癌已属晚期行去势术,加用氟至尔或缓退瘤口服治疗,术后分别于2~3年因全身广泛转移导致衰竭而死亡。结论:BPH电气化术腺后腺体残留或复发,主要为腺体切除不够为再手术主要原因;尿道狭窄主要原因为电凝止血时间长、电流大、创面烧灼严重;继发性出血主要发生前列腺窝创面结痂感染脱落所致。

The analysis for reason of second hospitalization after TUVP

Objective:Approach the reason of secondary hospitalization after TUVP.Methods:review and analysis 17 patients' clinical data,the reason of secondary hospitalization after TUVP concluded:16 glandular remains,5 urethral strictures,6 secondary hemorrhages,2 prostate cancers.Results:All of the glandular reviews or recurs have been TUVP,the symptom of urinary tract obstruction was relieved after TUVP.Among 5 urethral strictures,2 patients have been through ureter cryo-scalpel cut-off and vaporization of cicatricial tissue.2 patients simplely been cryo-scalpel cut-off of urethral strictures,1 patient cut-off of external urethral orifice.6 secondary hemorrhages have been examined cystoscope and rinsed blood clot after secondary hospitalization,and put three-vent urethral catheter into the bladder to draught and oppressed for hemostasis.Owing to be advanced stage,2 prostate cancers have been had castration operation,and applied androgen antagon,both patients died of exhaustion of all organs caused by cancer cells generally aversion.Conclusions:Review or recur of the alandular after TUVP was mainly caused by glandular been insufficiently cutted,urethral stricture was mainly caused by longly electric coagulation,large current,wound surface Severely cauterized,secondary hemorrhage was mainly caused by infected crust been ablated in prostate fossa.
